Management is horribly disorganized. Processes are poorly documented or not documented at all, and almost never followed, which causes endless problems in an FDA regulated industry. This leads to a perpetual panic mode where engineers are forced to rush out fixes instead of write quality software. There are interesting problems to solve here but you'll be too busy putting out yet another fire to ever work on them.
